It is very important for us to have your medical records prior to your first visit at Anderson Family Care. This will allow staff time to input the files into your chart and will give Dr. Anderson the opportunity to review your records prior to your visit.

As healthcare professionals, we make the most informed decisions when we have your complete past medical history available to us. We would like to have records from past primary care providers and all specialists you have seen in the past.

Medical records release forms are available in our office (and at the link below). Similar forms can be obtained and completed at your previous healthcare providers' offices as well.

If our completed and signed forms are returned to our office, we will be happy to send the request to the listed provider to obtain the records. If you request records from the offices of other providers, please kindly ask them to fax the records to us at 334-654-5081.
